Delaware State Innovation Model (DE SIM) State-Led Evaluation Report for AY2

The purpose of this report is to summarize the activities and results of the state-led evaluation
of the first-year implementation (AY2) for the Delaware State Innovation Model (DE SIM). DE
SIM is a broad-based health system transformation effort funded by the Centers for Medicare
and Medicaid Innovation (CMMI) currently being implemented across 38 states and territories.
As an expectation of the overall DE SIM plan, the state-led evaluation is intended to engage
stakeholders in a continuous improvement approach to examining the processes and outcomes
of DE SIM. In collaboration with DE SIM stakeholders, the state-led evaluation is expected to
provide input on, track, and inform stakeholders of progress towards unique, state-specific
implementation milestones and model outcomes. In doing so, a feedback loop will be created
for Delaware to track implementation, make mid-course corrections, and meet program goals.

Policy Proposal to Achieve Health Care Cost Savings

At yesterday’s Health Care Commission meeting DHSS Sec Kara Odom Walker reviewed her policy proposal to achieve health care cost savings.

A copy of Dr. Walker’s powerpoint presentation is available here (6.2.17 Sec Walker health care costs proposal).  This is the beginning of her leadership effort to implement payment reform with quality improvement as the guiding principles.

Why this matters for patients:  The DE Center for Health Innovation (DCHI) has been implementing a multi year plan to increase adoption of Accountable Care

Organization (ACO) structures and payment reform models including increased care coordination among medical professionals for improving quality outcomes for patients with care treatment better coordinated and supported by payment model reform.

This is the new Secretary’s plan for achieving the goals that are the foundation for the DE Health Care Commission (DHCC) and DCHI work supported in part by the DE Health Information Network (DHIN)
